Cards4Life is
a wellness charting system designed by a registered nurse to facilitate
healthcare
communication. It links illness with wellness and serves as a communication
tool when interfacing with health professionals. Cards4Life.com and
the Women's Heart Foundation, a 501c3 charity, are collaborating
to make this form more readily available to the public in order to
enhance and simplify health care delivery. The information on the
forms is the same information that is collected by health care institutions
around the country. This collaboration represents an attempt to standardize
the way that health information is stored and collected in order
to make health care work better for you. The succinct information
is printed on one side only so that it can be easily faxed or copied

Store your personal health file in a magnetized Cards4Life Vinyl Holder
Vinyl holders for Cards4Life are available for purchase at $5 each

Cards4Life for Everyone
Complete a Cards4Life health file for every member of your family
and place each into his/her personal vinyl file holder. Keep
it updated and take it with you to every medical appointment.
Ask your health professional to help you update it because its
about YOU and you are the end-user and are to be considered
the correct source for healthcare information about YOU. Store
each individual's Cards4Life file in its own personal vinyl file
holder in a file drawer with your other important papers. For
seniors who live along, it may be especially useful to affix
the file to a metal surface, such as the front of a refrigerator,
by applying the furnished magnet with adhesive backing. The vinyl
holder protects your medical information so that it can easily
be transported in purse or glove box.
Use a #2 PENCIL when completing Cards4Life for those areas subject
to frequent change. Please PRINT all information. Print your name
on the front, upper left panel. This page should be folded so that
it is easily visible when the card is placed into the file holder.
